WholeYouNYC helps New Yorkers find the resources and support they need to live healthy lives. We partner with trusted community organizations across New York City to help you every step of the way.

Our goal is to make it easier for you to access the services and resources that fit your needs. Anyone can use WholeYouNYC to connect with local services in their community.

When you come to us for assistance, you can trust that the information you provide will be kept completely confidential, and this service is entirely free.

Social Care Network

Expand access to high-quality Health-Related Social Needs (HRSN) services

Promote reliable screening via the "Accountable Health Communities" (AHC) screening tool.

Ensure transparency throughout the entire process for all members.

Strengthen collaboration between HRSN providers and partners.

Qualifying Support

If you live in Brooklyn, Queens, or Manhattan and are enrolled in Medicaid, you may also qualify for extra support.

  • Medically tailored food prescriptions

  • Food pantry stocking

  • Cooking Supplies

Transportation

  • Public or private transportation to social service agencies

Housing

  • Application assistance

  • Rent and utility assistance

  • Home safety modifications

  • Employment opportunities

  • Resume preparation assistance

Enhanced Health-Related
Social Needs

Medicaid Managed Care members can be navigated to existing benefit programs or referred via the Social Care Network to Enhanced Health-Related Social Needs services.

Medicaid Fee-for-Service members can receive navigation services to existing federal, state, or local benefit programs.

Enhanced Health-Related Social Needs Criteria

  • Unmet needs identified via NYS standardized screening.

  • Enrolled in Medicaid Managed Care.

  • Belongs to an eligible target group.

  • Meets specific clinical or social criteria (if applicable)

  • Frequent ER visits or hospitalizations (past 12 months).

  • Enrolled in Medicaid Health Home.

  • Substance Use Disorder or Serious Mental Illness.

  • Intellectual or Developmental Disability requiring support.

  • Pregnant or postpartum (past 12 months).

  • Released from incarceration (past 90 days) with health needs.

  • Under 18 with risk of chronic physical, behavioral, or emotional conditions.
